Takeda Receives Positive CHMP Opinion for Subcutaneous Formulation of Vedolizumab for use as Maintenance Therapy in Adults with Moderately to Severely Active Ulcerative Colitis or Crohn’s Disease

− If approved, the additional treatment modality will provide more options for how patients in Europe receive the gut-selective biologic vedolizumab

(BUSINESS WIRE)-- Takeda Pharmaceutical Company Limited (TSE:4502/NYSE:TAK) (“Takeda”) today announced that the Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) has recommended the approval of a subcutaneous (SC) formulation of the gut-selective biologic vedolizumab for use as maintenance therapy in adults with moderately to severely active ulcerative colitis (UC) or Crohn’s disease (CD). Takeda proposes to make vedolizumab SC available in both a pre-filled syringe and a pre-filled pen.

The CHMP opinion will now be reviewed by the European Commission. If approved, vedolizumab will become the only maintenance therapy for UC or CD with both intravenous and subcutaneous formulations across the European Union, providing greater options for how patients receive their treatment.

The positive opinion from the CHMP was based on the pivotal phase 3 VISIBLE trials which assessed the safety and efficacy of a SC formulation of vedolizumab as maintenance therapy in adult patients with moderately to severely active UC or CD who achieved clinical response* at week 6 following two doses of open-label vedolizumab intravenous (IV) therapy at weeks 0 and 2.1,2,3 Data from a long-term, open-label extension study of patients from VISIBLE 1 and VISIBLE 2 was also considered.4

* VISIBLE 1 UC: Clinical response is defined as a reduction in complete Mayo score of ≥3 points and ≥30% from baseline (week 0) with an accompanying decrease in rectal bleeding subscore of ≥1 point or absolute rectal bleeding subscore of ≤1 point.2

VISIBLE 2 CD: Clinical response is defined as a ≥70 point decrease in Crohn's Disease Activity Index (CDAI) score from baseline (week 0).5

About the VISIBLE Clinical Trial Program
 
The VISIBLE clinical trial program aims to assess the efficacy and safety of an investigational subcutaneous (SC) formulation of vedolizumab as maintenance therapy in adult patients with moderately to severely active ulcerative colitis (UC) or Crohn’s disease (CD).

The VISIBLE program consists of three phase 3 studies involving over 1,000 UC and CD patients which includes two randomized, double-blind, placebo-controlled studies examining the proportion of patients achieving clinical remission at week 52, and an open-label extension study to determine the long-term safety and efficacy of vedolizumab SC.2,3,4

About Ulcerative Colitis and Crohn’s Disease
 
Ulcerative colitis (UC) and Crohn’s disease (CD) are two of the most common forms of inflammatory bowel disease (IBD).Both UC and CD are chronic, relapsing, remitting, inflammatory conditions of the gastrointestinal tract, with CD potentially progressing over time.7,8 UC only involves the large intestine as opposed to CD which can affect any part of the GI tract from mouth to anus.9,10 CD can also affect the entire thickness of the bowel wall while UC only involves the innermost lining of the large intestine.9,10 UC commonly presents with symptoms of abdominal discomfort, loose bowel movements, including blood or pus.9,11 CD commonly presents with symptoms of abdominal pain, diarrhea, and weight loss.7 The cause of UC or CD is not fully understood; however, recent research suggests hereditary, genetics, environmental factors, and/or an abnormal immune response to microbial antigens in genetically predisposed individuals can lead to UC or CD.9,12,13

About Entyvio® (vedolizumab)
 
Vedolizumab is a gut-selective biologic and is approved as an intravenous (IV) formulation.14,15 It is a humanized monoclonal antibody designed to specifically antagonize the alpha4beta7 integrin, inhibiting the binding of alpha4beta7 integrin to intestinal mucosal addressin cell adhesion molecule 1 (MAdCAM-1), but not vascular cell adhesion molecule 1 (VCAM-1).16 MAdCAM-1 is preferentially expressed on blood vessels and lymph nodes of the gastrointestinal tract.17 The alpha4beta7 integrin is expressed on a subset of circulating white blood cells.16 These cells have been shown to play a role in mediating the inflammatory process in ulcerative colitis (UC) and Crohn’s disease (CD).16,18,19 By inhibiting alpha4beta7 integrin, vedolizumab may limit the ability of certain white blood cells to infiltrate gut tissues.16

Vedolizumab IV is approved for the treatment of adult patients with moderately to severely active UC and CD, who have had an inadequate response with, lost response to, or were intolerant to either conventional therapy or a tumor necrosis factor-alpha (TNFα)-antagonist.14,15 Vedolizumab IV has been granted marketing authorization in over 60 countries, including the United States and European Union, with more than 415,000 patient years of exposure to date.5

Therapeutic Indications for vedolizumab IV

Ulcerative colitis
Vedolizumab is indicated for the treatment of adult patients with moderately to severely active ulcerative colitis who have had an inadequate response with, lost response to, or were intolerant to either conventional therapy or a tumor necrosis factor-alpha (TNFα) antagonist.

Crohn’s disease
Vedolizumab is indicated for the treatment of adult patients with moderately to severely active Crohn’s disease who have had an inadequate response with, lost response to, or were intolerant to either conventional therapy or a tumor necrosis factor-alpha (TNFα) antagonist.

Important Safety Information for vedolizumab IV

Contraindications
Hypersensitivity to the active substance or to any of the excipients.

Special warnings and special precautions for use
 
Vedolizumab should be administered by a healthcare professional prepared to manage hypersensitivity reactions, including anaphylaxis, if they occur. Appropriate monitoring and medical support measures should be available for immediate use when administering vedolizumab. Observe patients during infusion and until the infusion is complete.

Infusion-related reactions
 
In clinical studies, infusion-related reactions (IRR) and hypersensitivity reactions have been reported, with the majority being mild to moderate in severity. If a severe IRR, anaphylactic reaction, or other severe reaction occurs, administration of vedolizumab must be discontinued immediately and appropriate treatment initiated (e.g., epinephrine and antihistamines). If a mild to moderate IRR occurs, the infusion rate can be slowed or interrupted and appropriate treatment initiated (e.g., epinephrine and antihistamines). Once the mild or moderate IRR subsides, continue the infusion. Physicians should consider pre-treatment (e.g., with antihistamine, hydrocortisone and/or paracetamol) prior to the next infusion for patients with a history of mild to moderate IRR to vedolizumab, in order to minimize their risks.

Infections
 
Vedolizumab is a gut-selective integrin antagonist with no identified systemic immunosuppressive activity. Physicians should be aware of the potential increased risk of opportunistic infections or infections for which the gut is a defensive barrier. Vedolizumab treatment is not to be initiated in patients with active, severe infections such as tuberculosis, sepsis, cytomegalovirus, listeriosis, and opportunistic infections until the infections are controlled, and physicians should consider withholding treatment in patients who develop a severe infection while on chronic treatment with vedolizumab. Caution should be exercised when considering the use of vedolizumab in patients with a controlled chronic severe infection or a history of recurring severe infections. Patients should be monitored closely for infections before, during and after treatment. Before starting treatment with vedolizumab, screening for tuberculosis may be considered according to local practice. Some integrin antagonists and some systemic immunosuppressive agents have been associated with progressive multifocal leukoencephalopathy (PML), which is a rare and often fatal opportunistic infection caused by the John Cunningham (JC) virus. By binding to the α4β7 integrin expressed on gut-homing lymphocytes, vedolizumab exerts an immunosuppressive effect specific to the gut. Although no systemic immunosuppressive effect was noted in healthy subjects, the effects on systemic immune system function in patients with inflammatory bowel disease are not known. Healthcare professionals should monitor patients on vedolizumab for any new onset or worsening of neurological signs and symptoms, and consider neurological referral if they occur. If PML is suspected, treatment with vedolizumab must be withheld; if confirmed, treatment must be permanently discontinued. Typical signs and symptoms associated with PML are diverse, progress over days to weeks, and include progressive weakness on one side of the body, clumsiness of limbs, disturbance of vision, and changes in thinking, memory, and orientation leading to confusion and personality changes. The progression of deficits usually leads to death or severe disability over weeks or months.

Malignancies
 
The risk of malignancy is increased in patients with ulcerative colitis and Crohn’s disease. Immunomodulatory medicinal products may increase the risk of malignancy.

Prior and concurrent use of biological products
 
No vedolizumab clinical trial data are available for patients previously treated with natalizumab. No clinical trial data for concomitant use of vedolizumab with biologic immunosuppressants are available. Therefore, the use of vedolizumab in such patients is not recommended.

Vaccinations
 
Prior to initiating treatment with vedolizumab all patients should be brought up to date with all recommended immunizations. Patients receiving vedolizumab may receive non-live vaccines (e.g., subunit or inactivated vaccines) and may receive live vaccines only if the benefits outweigh the risks.
Adverse reactions include: nasopharyngitis, headache, arthralgia, upper respiratory tract infection, bronchitis, influenza, sinusitis, cough, oropharyngeal pain, nausea, rash, pruritus, back pain, pain in extremities, pyrexia, fatigue and anaphylaxis.

تعتبر المرحلة الثالثة من تجربة "ألتا-1 إل"، (تجربة استخدام "بريجاتينيب" في علاج سرطان الرئة مع مورثة "إيه إل كي" في الخط الأول) لدراسة استخدام علاج "ألونبريج" لدى المرضى البالغين، تجربة مقارنة عالمية مستمرة وعشوائية مفتوحة العلامة ومتعددة المراكز، شارك فيها 275 مريضاً ("ألونبريج"، العدد= 137 مريضاً، "كريزوتينيب"، العدد= 138 مريضاً) يعانون من حالات متقدمة موضعياً أو نقيلية من سرطان الرئة غير صغير الخلايا الذي يعطي نتائج إيجابية لفحص مورثة "إيه إل كي" ولم يتلقوا علاجاً مسبقاً باستخدام مثبطات "إيه إل كي". وتلقّى المرضى إمّا 180 ميليجرام من "ألونبريج" مرة يومياً أو 90 ميليجرام مرة يومياً مع علاج مسبق لمدة سبعة أيام بجرعة 90 ميليجرام مرة يومياً، أو 250 ميليجرام من علاج "كريزوتينيب" مرتين يومياً.

وكان متوسط العمر 58 عاماً في ذراع "ألونبريج" و60 عاماً في ذراع "كريزوتينيب". وكان 29 في المئة من المرضى يعانون من النقيلات الدماغية بالحد الأدنى في ذراع "ألونبريج" مقابل 30 في المائة في ذراع "كريزوتينيب". تلقى 26 في المائة من المرضى علاجاً كيمياوياً مسبقاً للمرض المتقدم أو النقيلي في ذراع "ألونبريج" مقابل 27 في المائة في ذراع "كريزوتينيب".

وكانت نسبة البقاء على قيد الحياة دون تطور المرض بحسب تقييم لجنة المراجعة المستقلة المعمية هي نقطة النهاية الأولية. وشملت نقاط النهاية الثانوية معدل استجابة موضوعي وفق معايير تقييم الاستجابة في الأورام الصلبة "ريسيست النسخة 1.1"، ومدة الاستجابة داخل الجمجمة، والبقاء على قيد الحياة دون تطور المرض داخل الجمجمة، ومعدل البقاء على قيد الحياة الكلي والسلامة والتحمل.

وكان ملف السلامة لـ "ألونبريج" في تجربة "ألتا-1 إل" متسقاً بشكل عام مع معلومات تقديم الوصفات الطبية القائمة في الولايات المتحدة.

يُعتبر "ألونبريج" مثبط تيروزين كيناز قوي الفعالية وانتقائي جرى تصميمه لاستهداف ومنع التغييرات الوراثية في مورثة "إي إل كي". وتلقى "ألونبريج"، في أبريل 2017، موافقة مسرّعة من إدارة الغذاء والدواء لعلاج مرضى سرطان الرئة غير صغير الخلايا النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، والذين تطور مرضهم عند علاجهم بـ"كريزوتينيب"، أو الذين تُبدي أورامهم مقاومة للـ"كريزوتينيب". وتمت الموافقة على هذا الاستخدام ضمن حالة الموافقة المسرّعة على أساس معدل استجابة الورم ومدة الاستجابة. وقد يتوقف استمرار الموافقة على هذا الاستخدام على التحقّق من المزايا السريرية ووصفها في تجربة تأكيدية.

    المرحلة المحورية الثانية من تجربة "ألتا" للتحقق من مدى فعالية وسلامة "ألونبريج" في نظامين من الجرعات لعلاج مرضى سرطان الرئة غير صغير الخلايا المتقدم موضعياً أو النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، والذين تطور مرضهم عند علاجهم بـ"كريزوتينيب". أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثالثة من تجربة "ألتا-1 إل" العالمية العشوائية لتقييم مدى فعالية وسلامة "ألونبريج" بالمقارنة مع "كريزوتينيب" لعلاج مرضى سرطان الرئة غير صغير الخلايا المتقدم موضعياً أو النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، ولم يتلقوا علاجاً مسبقاً باستخدام مثبطات "إيه إل كي". أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثانية من تجربة "ألتا – جاي" وهي دراسة منفردة الأذرع متعددة المراكز لدى المرضى اليابانيين المصابين بسرطان الرئة غير صغير الخلايا النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، مع التركيز على المرضى الذين تطور مرضهم عند علاجهم بـ"أليكتينيب". أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثانية من دراسة "ألتا -2" العالمية منفردة الأذرع لتقييم "ألونبريج" لدى مرضى سرطان الرئة غير صغير الخلايا النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، والذين تطور مرضهم عند علاجهم بـ"أليكتينيب" و"سيريتينيب". أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثالثة من تجربة "ألتا -3" العالمية العشوائية لتقييم مدى فعالية وسلامة "ألونبريج" بالمقارنة مع "أليكتينيب" لعلاج مرضى سرطان الرئة غير صغير الخلايا النقيلي الذي يعطي نتيجة إيجابية لفحص مورثة "إيه إل كي"، والذين تطور مرضهم عند علاجهم بـ"كريزوتينيب". يجري حالياً تسجيل الراغبين بالمشاركة في هذه التجربة.

يعتبر "تي إيه كي- 788" مثبط انتقائي لطفرات مستقبل عامل النمو البشري (إي جي إف آر)/ مُستَقبِل عامل النمو البشري لدى الإنسان 2 ("إتش إي آر 2") ويتم استكشافه حالياً في طفرات لعمليات إدخال "إكزون 20" على مستقبل عامل النمو البشري "إي جي إف آر" :

    المرحلة 2/1 من الدراسة التي تقيم السلامة، والحرائك الدوائية والنشاط المضاد للأورام لمثبط مُستَقبِل عامل النمو البشري (إي جي إف آر)/ مُستَقبِل عامل النمو البشري لدى الإنسان 2 ("إتش إي آر 2") "تي إيه كي-788" لدى المرضى الذين يعانون من سرطان الرئة غير صغير الخلايا. أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثانية من "إكسكليم" المحورية للمجموعة الإضافية من تجربة المرحلة 2/1، التي صُممت لتقييم فعالية وسلامة "تي إيه كي-788" بجرعة 160 ميليجرام مرة يومياً لدى المرضى الذين خضعوا لعلاج سابقاً باستخدام طفرات لعمليات إدخال "إكزون 20" على مستقبل عامل النمو البشري "إي جي إف آر". أتمت هذه التجربة مرحلة تسجيل الراغبين بالمشاركة.
    المرحلة الثالثة من "إكسكليم 2" وهي دراسة عالمية عشوائية تقيم فعالية "تي إيه كي-788" كعلاج أولي بالمقارنة مع العلاج الكيميائي المزدوج القائم على البلاتين في المرضى الذين لم يخضعوا لأي علاج سابقاً ويعانون من حالات متقدمة موضعياً أو نقيلية من سرطان الرئة غير صغير الخلايا الذي يعطي نتائج إيجابية لفحص مورثة "إيه إل كي" وتحتوي أورامهم على طفرات لعمليات إدخال "إكزون 20" على مستقبل عامل النمو البشري "إي جي إف آر". يجري حالياً تسجيل الراغبين بالمشاركة في هذه التجربة.

    المرحلة الأولى من دراسة مفتوحة التسمية تمتد على مرحلتين وثابتة التسلسل تم تصميمها لإيجاد التفاعلات بين دواء وآخر بين "تي إيه كي-788" وإما مثبط السيتوكروم بيه -450 (سي واي بيه) 3 إيه القوي، إيتراكونازول" (الجزء 1) أو محفز "سي واي بيه 3 إيه" قوي، "ريفامبين" (الجزء 2) لدى البالغين الأصحاء المشاركين في الدارسة. يجري حالياً تسجيل الراغبين بالمشاركة في هذه التجربة.

معلومات مهمة للسلامة لعقار "ألونبريج"

التحذيرات والاحتياطات

مرض الرئة الخلالي "آي إل دي"/الالتهاب الرئوي: وقعت تفاعلات ضائرة رئوية شديدة وخطرة على الحياة ومميتة تتوافق مع مرض الرئة الخلالي/الالتهاب الرئوي عند استخدام "ألونبريج". وفي تجارب "ألتا"، وقعت الإصابة بمرض الرئة الخلالي/الالتهاب الرئوي لدى 3.7 في المائة من المرضى في مجموعة جرعة 90 ميليجرام (90 ميليجرام لمرة واحدة يومياً) ولدى 9.1 في المائة من المرضى في مجموعة جرعة 90 - 180 ميليجرام (180 ميليجرام مرة واحدة يومياً عقب فترة أولية لمدة سبعة أيام بجرعة 90 ميليجرام مرة واحدة يومياً). وحدثت تفاعلات ضائرة متوافقة مع إمكانية الإصابة بمرض الرئة الخلالي/الالتهاب الرئوي بشكل مبكر (خلال 9 أيام من بدء العلاج بـ"ألونبريج"، كان متوسط ظهور الأعراض يومين) لدى 6.4 في المائة من المرضى، حيث حدثت تفاعلات ضائرة تراوحت بين الدرجة الثالثة والرابعة لدى 2.7 في المائة. يتوجب مراقبة الأعراض التنفسية الجديدة أو المتفاقمة (مثل ضيق التنفس والسعال وغيرها)، خاصة خلال الأسبوع الأول من بدء العلاج باستخدام "ألونبريج". وينبغي إيقاف العلاج بـ"ألونبريج" لدى أي مريض يعاني من أعراض تنفسية جديدة أو متفاقمة، وإجراء تقييم لمرض الرئة الخلالي/الالتهاب الرئوي أو مسببات أخرى للأعراض التنفسية (مثل الانسداد الرئوي، أو تطور الورم، أو الالتهابات الرئوية المُعدية). أما بالنسبة إلى الدرجة الأولى أو الثانية من مرض الرئة الخلالي/الالتهاب الرئوي، يجب إما مواصلة العلاج مع تخفيف جرعة "ألونبريج" بعد التعافي للحد الأدنى أو إيقاف العلاج بـ"ألونبريج" بشكل نهائي. ويجب إيقاف العلاج بـ"ألونبريج" بشكل نهائي للدرجة الثالثة أو الرابعة من مرض الرئة الخلالي/الالتهاب الرئوي أو معاودة الإصابة بالدرجة الأولى أو الثانية من مرض الرئة الخلالي/الالتهاب الرئوي.

ارتفاع ضغط الدم: في تجارب "ألتا"، تمّ تسجيل حدوث ارتفاع في ضغط الدم لدى 11 في المائة من المرضى الذين تلقوا "ألونبريج" في مجموعة جرعة 90 ميليجرام ولدى 21 في المائة من المرضى في مجموعة جرعة 90 - 180 ميليجرام. وتعرّض 5.9 في المائة من مجمل المرضى للدرجة الثالثة لارتفاع ضغط الدم. يجب التحكم بضغط الدم قبل العلاج باستخدام "ألونبريج". كما ينبغي مراقبة ضغط الدم بعد أسبوعين وبشكل شهري على الأقل بعد ذلك أثناء العلاج بـ"ألونبريج". يجب إيقاف "ألونبريج" للدرجة الثالثة من ارتفاع ضغط الدم على الرغم من العلاجات المثلى الخافضة لضغط الدم. يمكن مواصلة العلاج بـ"ألونبريج" بجرعة مخففة عند انخفاض أو تحسّن الحدة إلى الدرجة الأولى. ويجب النظر في إيقاف العلاج بـ"ألونبريج" بشكل نهائي عند ارتفاع ضغط الدم للدرجة الرابعة أو تكرر ارتفاع ضغط الدم من الدرجة الثالثة. يجب توخي الحذر عند وصف "ألونبريج" مع العوامل الخافضة لضغط الدم التي تسبب بطء القلب.

بطء القلب: يمكن حدوث بطء في القلب عند العلاج باستخدام "ألونبريج". وفي تجارب "ألتا"، لوحظ حدوث نبضات قلب بمعدل أقل من 50 نبضة في الدقيقة لدى 5.7 في المائة من المرضى في مجموعة جرعة 90 ميليجرام ولدى 7.6 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. وحدث بطء للقلب من الدرجة الثانية لدى مريض واحد (0.9 في المائة) في مجموعة جرعة 90 ميليجرام. يجب مراقبة معدل نبض القلب وضغط الدم خلال العلاج باستخدام "ألونبريج". يجب مراقبة المرضى بشكل متكرر إذا ترافق العلاج مع استخدام أدوية أخرى، لا يمكن تجنبها، تعرف بتسببها ببطء القلب. عند حدوث بطء عرضي للقلب، يجب إيقاف العلاج بـ"ألونبريج" ومراجعة الأدوية المصاحبة خاصة تلك المعروفة بتسببها ببطء القلب. إذا تم تحديد وإيقاف الدواء المصاحب الذي يُعرف بتسببه ببطء القلب أو تمّ تعديل الجرعة، يجب مواصلة العلاج باستخدام "ألونبريج" بنفس الجرعة عقب انخفاض أعراض بطء القلب، أما عند حصول خلاف ذلك، يجدر خفض جرعة "ألونبريج" عقب انخفاض أعراض بطء القلب. ويجب إيقاف "ألونبريج" عند حدوث بطء في القلب يشكل تهديداً على الحياة في حال لم يتم تحديد أي دواء مصاحب يسبب ذلك.

الاضطرابات البصرية: وفي تجارب "ألتا"، سُجّل حدوث تفاعلات ضائرة تؤدي إلى اضطرابات بصرية، تتضمن عدم وضوح الرؤية والشفع وانخفاض حدّة البصر، لدى 7.3 في المائة من المرضى الذين تمّ علاجهم باستخدام "ألونبريج" في مجموعة جرعة 90 ميليجرام ولدى 10 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. وحدث وذمة في البقعة الصفراء وإعتام لعدسة العين من الدرجة الثالثة لدى مريض واحد لكل من الحالتين في مجموعة جرعة 90 – 180 ميليجرام. يجب نصح المرضى بالإفصاح عن أي عرض في الرؤية، وينبغي إيقاف "ألونبريج" وإجراء تقييم لعيون للمرضى الذين يظهرون أعراضاً جديدة أو متفاقمة بحدّة من الدرجة الثانية أو أكثر. وعقب التعافي من الاضطرابات البصرية من الدرجتين الثانية أو الثالثة وانخفاضها إلى الدرجة الأولى أو للحد الأدنى، يجب مواصلة العلاج باستخدام "ألونبريج" بجرعة مخففة. ويجب إيقاف العلاج بـ"ألونبريج" بشكل نهائي لدى حدوث اضطرابات بصرية من الدرجة الرابعة.

ارتفاع مستوى أنزيم كرياتين فوسفوكيناز ("سي بيه كيه"): في تجارب "ألتا"، حدث ارتفاع في مستوى أنزيم كرياتين فوسفوكيناز لدى 27 في المائة من المرضى الذين يتم علاجهم باستخدام "ألونبريج" في مجموعة جرعة 90 ميليجرام ولدى 48 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. وكانت نسبة حدوث ارتفاع مستوى أنزيم كرياتين فوسفوكيناز من الدرجة الثالثة أو الرابعة 2.8 في المائة في مجموعة جرعة 90 ميليجرام و12 في المائة في مجموعة جرعة 90 – 180 ميليجرام. وتمّ تخفيض الجرعة بسبب ارتفاع أنزيم كرياتين فوسفوكيناز لدى 1.8 في المائة من المرضى في مجموعة جرعة 90 ميليجرام و4.5 في المائة من مجموعة جرعة 90 – 180 ميليجرام. يجب نصح المرضى بالإفصاح عن أي آلام غير مبررة في العضلات أو وهن أو ضعف. ويجب مراقبة مستويات أنزيم كرياتين فوسفوكيناز أثناء المعالجة باستخدام "ألونبريج". وينبغي إيقاف العلاج بـ"ألونبريج" عند ارتفاع مستوى أنزيم كرياتين فوسفوكيناز للدرجة الثالثة أو الرابعة. وعند التعافي أو الانخفاض إلى الدرجة الأولى أو إلى الحدّ الأدنى، يجب مواصلة العلاج بـ"ألونبريج" بنفس الجرعة أو بجرعة مخفّفة.

ارتفاع مستوى أنزيمات البنكرياس: في تجربة "ألتا"، سُجّل ارتفاع مستوى أنزيم الأميلاز لدى 27 في المائة من المرضى في مجموعة جرعة 90 ميليجرام و39 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. كما ارتفع مستوى أنزيم الليباز لدى 21 في المائة من المرضى في مجموعة جرعة 90 ميليجرام و45 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. وحدث ارتفاع لمستوى الأميلاز من الدرجتين الثالثة أو الرابعة لدى 3.7 في المائة من المرضى في مجموعة جرعة 90 ميليجرام و2.7 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. هذا وحدث ارتفاع لمستوى الليباز من الدرجتين الثالثة أو الرابعة لدى 4.6 في المائة من المرضى في مجموعة جرعة 90 ميليجرام و5.5 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. ويُنصح بمراقبة مستويات أنزيمي الليباز والأميلاز أثناء العلاج بـ"ألونبريج" وإيقاف العلاج عند ارتفاع مستوى أنزيمات البنكرياس من الدرجتين الثالثة أو الرابعة. وعند التعافي أو الانخفاض إلى الدرجة الأولى أو إلى الحد الأدنى، يمكن استئناف تناول "ألونبريج" بنفس الجرعة أو بجرعة مخففة.

فرط سكر الدم: في تجربة "ألتا"، شهد 43 في المائة من المرضى الخاضعين لعلاج "ألونبريج" ارتفاعاً جديداً أو متفاقماً في سكر الدم. وعانى 3.7 في المائة من المرضى من فرط سكر الدم من الدرجة الثالثة، وفق التقييم المخبري لمستويات الجلوكوز في الدم قبل الإفطار. وتطلّب مريضان، من أصل 20 مريضاً (10 في المائة) ممن يعانون من مرض السكري أو عدم تحمل الجلوكوز عند الحد الأدنى، علاجهم بالـ "أنسولين" أثناء العلاج بـ"ألونبريج". يجب تقييم مستوى الجلوكوز قبل الإفطار قبل العلاج بـ"ألونبريج" والمراقبة بشكل دوري بعد ذلك، ويُرجى إعطاء أو تعديل تناول الأدوية المخفضة لفرط سكر الدم حسب الحاجة. وعند استحالة التحكّم المناسب في نسبة سكر الدم مع الإدارة الطبية المثلى، ينبغي إيقاف "ألونبريج" حتى التمكّن من التحكم في نسبة سكر الدم والنظر في تقليل جرعة "ألونبريج" أو إيقاف العلاج بـ"ألونبريج" نهائياً.

التسمم الجنيني: استناداً إلى طريقة عمله ونتائجه على الحيوانات، يمكن أن يتسبب "ألونبريج" بضرر للجنين عند إعطائه لنساء حوامل. لا توجد بيانات سريرية حول استخدام "ألونبريج" لدى النساء الحوامل. وينبغي تنبيههنّ إلى المخاطر المحتملة على الجنين. وتُنصح المرأة القادرة على الإنجاب باستخدام وسائل منع الحمل الفعالة غير الهرمونية أثناء العلاج بـ"ألونبريج" ولمدة 4 أشهر على الأقل بعد الجرعة النهائية. كما يُنصح الرجال ذوي الشريكات القادرات على الإنجاب باستخدام وسائل منع الحمل الفعالة أثناء العلاج بـ"ألونبريج" ولمدة 3 أشهر على الأقل بعد الجرعة النهائية.

التفاعلات الضائرة

حدثت تفاعلات ضائرة خطيرة لدى 38 في المائة من المرضى في مجموعة جرعة 90 ميليجرام ونسبة 40 في المائة من المرضى في مجموعة جرعة 90 – 180 ميليجرام. وكانت التفاعلات الضائرة الخطيرة الأكثر شيوعاً هي الالتهاب الرئوي (5.5 في المائة من مجمل المرضى، و3.7 في المائة في مجموعة جرعة 90 ميليجرام، و7.3 في المائة في مجموعة جرعة 90 – 180 ميليجرام) والمرض الرئوي الخلالي/الالتهاب الرئوي (4.6 في المائة من مجمل المرضى، و1.8 في المائة في مجموعة جرعة 90 ميليجرام، و7.3 في المائة في مجموعة جرعة 90 – 180 ميليجرام). وحصلت تفاعلات ضائرة مميتة لدى 3.7 في المائة من المرضى وشملت الالتهاب الرئوي (مريضيَن)، والموت المفاجئ، وضيق النفس، وفشل الجهاز التنفسي، والانسداد الرئوي، والتهاب السحايا الجرثومي والانتان البولي (مريض واحد لكل حالة).

وكانت التفاعلات الضائرة الأكثر شيوعاً (25 في المائة أو أكثر) لدى مجموعة جرعة 90 ميليجرام: الغثيان (33 في المائة)، والإرهاق (29 في المائة)، والصداع (28 في المائة)، وضيق النفس (27 في المائة) وفي مجموعة جرعة 90 – 180 ميليجرام كانت الغثيان (40 في المائة)، والإسهال (38 في المائة) والإرهاق (36 في المائة)، والسعال (34 في المائة) والصداع (27 في المائة).

التفاعلات الدوائية

مثبطات سيتوكروم "سي واي بي 3 إيه": ينبغي تفادي الاستخدام المتزامن لـ"ألونبريج" مع مثبطات سيتوكروم "سي واي بي 3 إيه" القوية أو المعتدلة. يُنصح بعدم تناول فاكهة أو عصير الجريب فروت إذ قد تزيد أيضاً من تركيز "بريجاتينيب" في مصل الدّم. وإذا لم يكن هنالك مفرّ من الاستخدام المتزامن لمثبط سيتوكروم "سي واي بي 3 إيه" القوي أو المعتدل، يجدر تخفيض جرعة "ألونبريج".

محفزات سيتوكروم "سي واي بي 3 إيه": ينبغي تفادي الاستخدام المتزامن لـ"ألونبريج" مع المحفزات سيتوكروم "سي واي بي 3 إيه" القوية أو المعتدلة. وإن لم يكن هناك مفرّ من الاستخدام المتزامن لمحفزات سيتوكروم "سي واي بي 3 إيه"، يجب زيادة جرعة "ألونبريج".

ركائز سيتوكروم "سي واي بي 3 إيه": يمكن أن يؤدي الاستخدام المتزامن بـ"ألونبريج" مع ركائز سيتوكروم "سي واي بي 3 إيه" الحساسة، بما في ذلك وسائل منع الحمل الهرمونية، إلى انخفاض تركيزات وفقدان فعالية ركائز "سي واي بي 3 إيه" الحساسة.

الاستخدام لدى فئات معينة

الحمل: يمكن أن يسبب "ألونبريج" ضرراً للجنين. تُنصح النساء القادرات على الإنجاب حول المخاطر المحتملة على الجنين.

الرضاعة: لا تتوافر بيانات حول إفراز "بريجاتينيب" في الحليب البشري أو تأثيره على الرضع أثناء الرضاعة الطبيعية أو إنتاج الحليب. ونظراً للتفاعلات الضائرة المحتملة لدى الرضع أثناء الرضاعة الطبيعية، تُنصح المرأة المرضعة بإيقاف الإرضاع خلال الخضوع لعلاج بـ"ألونبريج".

النساء والرجال القادرون على الإنجاب:

اختبارات الحمل: يجب التأكد من حالة الحمل لدى النساء القادرات على الإنجاب قبل بدء العلاج بـ"ألونبريج".

وسائل منع الحمل: تُنصح المرأة القادرة على الإنجاب باستخدام وسائل منع حمل غير هرمونية فعالة أثناء العلاج بـ"ألونبريج" ولمدة 4 أشهر على الأقل بعد الجرعة النهائية. يُنصح الرجال ذوي الشريكات القادرات على الإنجاب باستخدام وسائل منع الحمل الفعالة أثناء العلاج بـ"ألونبريج" ولمدة 3 أشهر على الأقل بعد الجرعة النهائية.

العقم: قد يسبب "ألونبريج" انخفاض الخصوبة لدى الرجال.

الاستخدام لعلاج الأطفال: لم تُحدّد سلامة وفعالية استخدام "ألونبريج" لدى المرضى الأطفال.

الاستخدام لعلاج المسنين: لم تشمل الدراسات السريرية لـ"ألونبريج" أعداداً كافية من المرضى الذين تبلغ أعمارهم 65 عاماً أو أكثر لتحديد إمكانية تجاوبهم بشكل مختلف عن المرضى الأصغر سناً.

Takeda Announces U.S. FDA Grants Priority Review for Supplemental New Drug Application for ALUNBRIG® (brigatinib) as a First-Line Treatment for ALK+ Metastatic Non-Small Cell Lung Cancer

(BUSINESS WIRE)-- Takeda Pharmaceutical Company Limited (TSE:4502/NYSE:TAK) today announced that the U.S. Food and Drug Administration (FDA) has granted priority review for the company’s supplemental New Drug Application (sNDA) to expand the use of ALUNBRIG (brigatinib) as a first-line treatment for patients with anaplastic lymphoma kinase-positive (ALK+) metastatic non-small cell lung cancer (NSCLC) as detected by an FDA-approved test. ALUNBRIG is a next-generation tyrosine kinase inhibitor (TKI) that was designed to target and inhibit ALK genetic alterations.

The sNDA for ALUNBRIG as a first-line treatment is based on results from the Phase 3 ALTA-1L trial, which is evaluating the safety and efficacy of ALUNBRIG in patients with ALK+ locally advanced or metastatic NSCLC who have not received prior treatment with an ALK inhibitor compared to that of crizotinib in the same population. The ALTA-1L trial met its primary endpoint with ALUNBRIG demonstrating superiority in blinded independent review committee (BIRC)-assessed progression-free survival (PFS) compared to crizotinib.

About the ALTA-1L Trial

The Phase 3 ALTA-1L (ALK in Lung Cancer Trial of BrigAtinib in 1st Line) trial of ALUNBRIG in adults is a global, ongoing, randomized, open-label, comparative, multicenter trial, which enrolled 275 patients (ALUNBRIG, n=137, crizotinib, n=138) with ALK+ locally advanced or metastatic NSCLC who have not received prior treatment with an ALK inhibitor. Patients received either ALUNBRIG, 180 mg once daily with seven-day lead-in at 90 mg once daily, or crizotinib, 250 mg twice daily.

The median age was 58 years in the ALUNBRIG arm and 60 years in the crizotinib arm. Twenty-nine percent of patients had brain metastases at baseline in the ALUNBRIG arm versus 30% in the crizotinib arm. Twenty-six percent of patients received prior chemotherapy for advanced or metastatic disease in the ALUNBRIG arm versus 27% in the crizotinib arm.

Blinded independent review committee (BIRC)-assessed progression-free survival (PFS) was the primary endpoint. Secondary endpoints included objective response rate (ORR) per RECIST v1.1, intracranial ORR, intracranial PFS, overall survival (OS), safety and tolerability.

The safety profile of ALUNBRIG in the ALTA-1L trial was generally consistent with the existing U.S. prescribing information.

About ALUNBRIG® (brigatinib)

ALUNBRIG is a potent and selective next-generation tyrosine kinase inhibitor (TKI) that was designed to target and inhibit anaplastic lymphoma kinase (ALK) genetic alterations. In April 2017, ALUNBRIG received Accelerated Approval from the U.S. FDA for ALK+ metastatic NSCLC patients who have progressed on or are intolerant to crizotinib. This indication is approved under Accelerated Approval based on tumor response rate and duration of response. Continued approval for this indication may be contingent upon verification and description of clinical benefit in a confirmatory trial.

ALUNBRIG is currently approved in more than 40 countries, including the U.S., Canada and the European Union, for the treatment of people living with ALK+ metastatic NSCLC who have taken the medicine crizotinib, but their NSCLC has worsened or they cannot tolerate taking crizotinib.

ALUNBRIG received Breakthrough Therapy Designation from the FDA for the treatment of patients with ALK+ NSCLC whose tumors are resistant to crizotinib and was granted Orphan Drug Designation by the FDA for the treatment of ALK+ NSCLC, ROS1+ and EGFR+ NSCLC.

    Phase 1, open-label, multicenter, dose-escalation study evaluating the safety, tolerability and pharmacokinetics of TAK-788 in Japanese patients with locally advanced or metastatic NSCLC. This trial has completed enrollment.
    Phase 2 J-EXCLAIM, open-label, multicenter, study evaluating the efficacy of TAK-788 as a first-line treatment in Japanese patients with locally advanced or metastatic NSCLC whose tumors harbor EGFR exon 20 insertion mutations. This trial is now enrolling.
    Phase 1, open-label, two-period, fixed-sequence study designed to characterize drug-drug interaction between TAK-788 and either a strong cytochrome P-450 (CYP)3A inhibitor, itraconazole (Part 1) or a strong CYP3A inducer, rifampin (Part 2) in healthy adult subjects. ALUNBRIG IMPORTANT SAFETY INFORMATION

WARNINGS AND PRECAUTIONS

Interstitial Lung Disease (ILD)/Pneumonitis: Severe, life-threatening, and fatal pulmonary adverse reactions consistent with interstitial lung disease (ILD)/pneumonitis have occurred with ALUNBRIG. In Trial ALTA (ALTA), ILD/pneumonitis occurred in 3.7% of patients in the 90 mg group (90 mg once daily) and 9.1% of patients in the 90→180 mg group (180 mg once daily with 7-day lead-in at 90 mg once daily). Adverse reactions consistent with possible ILD/pneumonitis occurred early (within 9 days of initiation of ALUNBRIG; median onset was 2 days) in 6.4% of patients, with Grade 3 to 4 reactions occurring in 2.7%. Monitor for new or worsening respiratory symptoms (e.g., dyspnea, cough, etc.), particularly during the first week of initiating ALUNBRIG. Withhold ALUNBRIG in any patient with new or worsening respiratory symptoms, and promptly evaluate for ILD/pneumonitis or other causes of respiratory symptoms (e.g., pulmonary embolism, tumor progression, and infectious pneumonia). For Grade 1 or 2 ILD/pneumonitis, either resume ALUNBRIG with dose reduction after recovery to baseline or permanently discontinue ALUNBRIG. Permanently discontinue ALUNBRIG for Grade 3 or 4 ILD/pneumonitis or recurrence of Grade 1 or 2 ILD/pneumonitis.

Hypertension: In ALTA, hypertension was reported in 11% of patients in the 90 mg group who received ALUNBRIG and 21% of patients in the 90→180 mg group. Grade 3 hypertension occurred in 5.9% of patients overall. Control blood pressure prior to treatment with ALUNBRIG. Monitor blood pressure after 2 weeks and at least monthly thereafter during treatment with ALUNBRIG. Withhold ALUNBRIG for Grade 3 hypertension despite optimal antihypertensive therapy. Upon resolution or improvement to Grade 1 severity, resume ALUNBRIG at a reduced dose. Consider permanent discontinuation of treatment with ALUNBRIG for Grade 4 hypertension or recurrence of Grade 3 hypertension. Use caution when administering ALUNBRIG in combination with antihypertensive agents that cause bradycardia.

Bradycardia: Bradycardia can occur with ALUNBRIG. In ALTA, heart rates less than 50 beats per minute (bpm) occurred in 5.7% of patients in the 90 mg group and 7.6% of patients in the 90→180 mg group. Grade 2 bradycardia occurred in 1 (0.9%) patient in the 90 mg group. Monitor heart rate and blood pressure during treatment with ALUNBRIG. Monitor patients more frequently if concomitant use of drug known to cause bradycardia cannot be avoided. For symptomatic bradycardia, withhold ALUNBRIG and review concomitant medications for those known to cause bradycardia. If a concomitant medication known to cause bradycardia is identified and discontinued or dose adjusted, resume ALUNBRIG at the same dose following resolution of symptomatic bradycardia; otherwise, reduce the dose of ALUNBRIG following resolution of symptomatic bradycardia. Discontinue ALUNBRIG for life-threatening bradycardia if no contributing concomitant medication is identified.

Visual Disturbance: In ALTA, adverse reactions leading to visual disturbance including blurred vision, diplopia, and reduced visual acuity, were reported in 7.3% of patients treated with ALUNBRIG in the 90 mg group and 10% of patients in the 90→180 mg group. Grade 3 macular edema and cataract occurred in one patient each in the 90→180 mg group. Advise patients to report any visual symptoms. Withhold ALUNBRIG and obtain an ophthalmologic evaluation in patients with new or worsening visual symptoms of Grade 2 or greater severity. Upon recovery of Grade 2 or Grade 3 visual disturbances to Grade 1 severity or baseline, resume ALUNBRIG at a reduced dose. Permanently discontinue treatment with ALUNBRIG for Grade 4 visual disturbances.

Creatine Phosphokinase (CPK) Elevation: In ALTA, creatine phosphokinase (CPK) elevation occurred in 27% of patients receiving ALUNBRIG in the 90 mg group and 48% of patients in the 90 mg→180 mg group. The incidence of Grade 3-4 CPK elevation was 2.8% in the 90 mg group and 12% in the 90→180 mg group. Dose reduction for CPK elevation occurred in 1.8% of patients in the 90 mg group and 4.5% in the 90→180 mg group. Advise patients to report any unexplained muscle pain, tenderness, or weakness. Monitor CPK levels during ALUNBRIG treatment. Withhold ALUNBRIG for Grade 3 or 4 CPK elevation. Upon resolution or recovery to Grade 1 or baseline, resume ALUNBRIG at the same dose or at a reduced dose.

Pancreatic Enzyme Elevation: In ALTA, amylase elevation occurred in 27% of patients in the 90 mg group and 39% of patients in the 90→180 mg group. Lipase elevations occurred in 21% of patients in the 90 mg group and 45% of patients in the 90→180 mg group. Grade 3 or 4 amylase elevation occurred in 3.7% of patients in the 90 mg group and 2.7% of patients in the 90→180 mg group. Grade 3 or 4 lipase elevation occurred in 4.6% of patients in the 90 mg group and 5.5% of patients in the 90→180 mg group. Monitor lipase and amylase during treatment with ALUNBRIG. Withhold ALUNBRIG for Grade 3 or 4 pancreatic enzyme elevation. Upon resolution or recovery to Grade 1 or baseline, resume ALUNBRIG at the same dose or at a reduced dose.

Hyperglycemia: In ALTA, 43% of patients who received ALUNBRIG experienced new or worsening hyperglycemia. Grade 3 hyperglycemia, based on laboratory assessment of serum fasting glucose levels, occurred in 3.7% of patients. Two of 20 (10%) patients with diabetes or glucose intolerance at baseline required initiation of insulin while receiving ALUNBRIG. Assess fasting serum glucose prior to initiation of ALUNBRIG and monitor periodically thereafter. Initiate or optimize anti-hyperglycemic medications as needed. If adequate hyperglycemic control cannot be achieved with optimal medical management, withhold ALUNBRIG until adequate hyperglycemic control is achieved and consider reducing the dose of ALUNBRIG or permanently discontinuing ALUNBRIG.

Embryo-Fetal Toxicity: Based on its mechanism of action and findings in animals, ALUNBRIG can cause fetal harm when administered to pregnant women. There are no clinical data on the use of ALUNBRIG in pregnant women. Advise pregnant women of the potential risk to a fetus. Advise females of reproductive potential to use effective non-hormonal contraception during treatment with ALUNBRIG and for at least 4 months following the final dose. Advise males with female partners of reproductive potential to use effective contraception during treatment and for at least 3 months after the last dose of ALUNBRIG.

ADVERSE REACTIONS

Serious adverse reactions occurred in 38% of patients in the 90 mg group and 40% of patients in the 90→180 mg group. The most common serious adverse reactions were pneumonia (5.5% overall, 3.7% in the 90 mg group, and 7.3% in the 90→180 mg group) and ILD/pneumonitis (4.6% overall, 1.8% in the 90 mg group and 7.3% in the 90→180 mg group). Fatal adverse reactions occurred in 3.7% of patients and consisted of pneumonia (2 patients), sudden death, dyspnea, respiratory failure, pulmonary embolism, bacterial meningitis and urosepsis (1 patient each).

The most common adverse reactions (≥25%) in the 90 mg group were nausea (33%), fatigue (29%), headache (28%), and dyspnea (27%) and in the 90→180 mg group were nausea (40%), diarrhea (38%), fatigue (36%), cough (34%), and headache (27%).

DRUG INTERACTIONS

CYP3A Inhibitors: Avoid coadministration of ALUNBRIG with strong or moderate CYP3A inhibitors. Avoid grapefruit or grapefruit juice as it may also increase plasma concentrations of brigatinib. If coadministration of a strong or moderate CYP3A inhibitor cannot be avoided, reduce the dose of ALUNBRIG.

CYP3A Inducers: Avoid coadministration of ALUNBRIG with strong or moderate CYP3A inducers. If coadministration of moderate CYP3A inducers cannot be avoided, increase the dose of ALUNBRIG.

CYP3A Substrates: Coadministration of ALUNBRIG with sensitive CYP3A substrates, including hormonal contraceptives, can result in decreased concentrations and loss of efficacy of sensitive CYP3A substrates.

USE IN SPECIFIC POPULATIONS

Pregnancy: ALUNBRIG can cause fetal harm. Advise females of reproductive potential of the potential risk to a fetus.

Lactation: There are no data regarding the secretion of brigatinib in human milk or its effects on the breastfed infant or milk production. Because of the potential adverse reactions in breastfed infants, advise lactating women not to breastfeed during treatment with ALUNBRIG.

Females and Males of Reproductive Potential:

Pregnancy Testing: Verify pregnancy status in females of reproductive potential prior to initiating ALUNBRIG.

Contraception: Advise females of reproductive potential to use effective non-hormonal contraception during treatment with ALUNBRIG and for at least 4 months after the final dose. Advise males with female partners of reproductive potential to use effective contraception during treatment with ALUNBRIG and for at least 3 months after the final dose.

Infertility: ALUNBRIG may cause reduced fertility in males.

Pediatric Use: The safety and effectiveness of ALUNBRIG in pediatric patients have not been established.

Geriatric Use: Clinical studies of ALUNBRIG did not include sufficient numbers of patients aged 65 years and older to determine whether they respond differently from younger patients.

Hepatic or Renal Impairment: No dose adjustment is recommended for patients with mild or moderate hepatic impairment or mild or moderate renal impairment. Reduce the dose of ALUNBRIG for patients with severe hepatic impairment or severe renal impairment.
